Zorgfuik keeps movingFrom five to nine checks
The expansion follows quickly after the first five checks went live, and that is deliberate. While refining the first set, four recurring process problems stood out as deserving a short check of their own. Keeping them separate means each questionnaire can stay focused and easy to use.
Care coordination check
Who oversees the pathway as a whole? This check looks at overview, ownership, follow-up and whether the person coordinating can actually act when the pathway stalls.
Waiting period check
This is about more than the number of weeks or months. It asks what happens while someone waits: who remains responsible, whether changes are monitored and whether reassessment is possible when circumstances change.
Care handover check
A referral does not automatically mean a handover is complete. This check looks at whether information and responsibility actually reached the next provider, whether continuity was maintained and who fixes a handover that goes wrong.
Access to care check
Sometimes the problem happens before a pathway has even started. This check maps the route into care or support, criteria and explanations, practical barriers, alternatives and who remains responsible when access fails.
